By proceeding with any esthetician services provided by On The Glow, you agree to the terms outlined below. This consent form is designed to inform you about the procedures and help you understand the potential impacts.

Consent to Treatment

By agreeing to receive esthetics services at On The Glow, you confirm that:

• You are over 18 years of age or have parental consent if under 18.
• You have disclosed any allergies, skin conditions, or current medications that may affect treatment.
• You understand that esthetics services are not a substitute for medical care.
• You acknowledge that results may vary from person to person.

Potential Risks

During facial treatments, you may experience some of the following temporary side effects:

• Redness or skin irritation
• Breakouts or minor swelling
• In rare cases, an allergic reaction

Contraindications to Facial treatments

• Active infections,: bacterial, viral cold sores, shingles, flu, COVID, or fungal ringworm
• Contagious illness
• Recent Chemotherapy
• use of accutane within the last 12 months
• Broken or burned skin ; ie sun burn, active eczema flares
• Recent cosmetic procedures; botox/ chemical peels must wait 14 days
• Uncontrolled Diabetes
• Use of retinol within 72 hrs prior of facial

Please inform the esthetician immediately if you experience any discomfort during the treatment.

Personal Responsibilities

As a client, you are responsible for:

1. Providing accurate and updated health information.
2. Following pre- and post-treatment instructions given by the esthetician.
3. Communicating openly with the esthetician about any concerns or questions.

Release of Liability

By receiving services at On The Glow, you release the business, the estheticians, and any affiliated staff from any liability and assume full responsibility for any adverse reactions or conditions arising from the esthetics services received.

Cancellation Policy

Kindly provide at least 24 hours' notice if you need to cancel or reschedule your appointment to avoid cancellation fees.
